Non-Medicare patients with complex medical history

$125 per month

$1,375 if paid upfront annually — ANNUAL SAVINGS $125

Primary Care- Plus:

Non-Medicare patients without complex medical history

$75 per month

$825 if paid upfront annually — ANNUAL SAVINGS $75
